This page describes the current research website. It does not describe a launched clinical service or authorize medical-record uploads. These factual notes are not a substitute for a reviewed privacy policy.

Survey answers

The survey uses predefined choices about care experiences, broad data-use categories, workarounds, spending bands and preferences. It asks for no name, email, diagnosis, medication name, lab value or medical record. Broad health-related answers can still be personal information.

Completed submissions are stored with a generated response ID, submission time and referral source. Eligibility is checked before submission. The application does not provide a partial-answer save flow.

Optional contact details

The separate interview form collects first name, email, US time zone, broad research group, availability and consent choices. These are stored in a separate contact table with a generated ID, submission time and referral source.

The availability field is for scheduling only. Do not put health information in it. The application does not automatically detect or remove medical details entered there.

Separation and anonymity

The survey and contact tables have no direct response-to-contact link. That does not guarantee anonymity. Submission times, referral sources and technical metadata can still permit identification or correlation.

Hosting and access

The website's hosting and database infrastructure processes submissions. The survey handler does not add IP addresses or browser details to its response table, but the hosting service may process or retain technical request metadata. We have not yet verified all provider logging, administrative access, storage locations or subprocessors for a public privacy notice.

Retention and questions

No automatic deletion schedule is implemented in the application, and a reviewed retention period has not been published. Provider logs, backups and manual deletion practices have not been verified in this review. If you need those details before participating, contact cody@getstrandhealth.com.
